Mission

The Colorado Primary Care Collaborative (CPCC) is dedicated to advancing primary care via the patient-centered medical home (PCMH) and other value-based primary care models by focusing on delivery reform, payment reform, patient engagement, workforce training, and benefit redesign.

Values

• Partnerships: The CPCC partners with the national PCPCC and establishes partnerships with providers and consumers of health care.
• Advocacy: The CPCC achieves its local mission by advocating for and promoting primary care through support of the PCMH model, payment reform, and practice transformation to patients, businesses, health plans, and governments.
• Quality health care: The CPCC values health care that is accessible, cost-effective, and demonstrates quality outcomes consistent with Triple Aims: cost effective, patient experience, and quality.
• Patient engagement: The CPCC champions the involvement of patients and families in their health care.

Activities

• To achieve our mission, we work in partnership with our members and fellow primary care advocates to promote the vision of the CPCC to patients, purchasers, businesses, health plans and government by:
• Disseminating results and outcomes from medical home initiatives and clearly communicating their impact on patient experience, quality of care, population health and health care costs.
• Advocating for public policy that advances and builds support for primary care and the medical home, including payment reform, patient engagement, employer benefit initiatives, and training the primary care workforce.
• Convening health care experts, thought leaders, and consumers to promote learning, awareness, and innovation of comprehensive and coordinated primary care services and the medical home model.
